Initial Rehabilitation Assessment

If required, this assessment is conducted at the time of referral and includes:

• Evaluation of the injured workers status including their medical status, treatment plan & specific needs.
• Clearly articulated understanding of the workplace and the employment structure.
  Identification and communication of the suitable employment goal and the strategies to achieve the goal.
• Identification of any barriers to achieving return to work and strategies to overcome such barriers.
• Determining the appropriate services and activities to be included in the workers return to work plan.
• A comprehensive written report is provided.
